Declaration of consent for data collection for the calculation of 4eye activation codes


The PCI regulations to be applied within the electronic payment transactions stipulate that the following personal data must be collected und stored for the calculation of activation codes for the commissioning of the terminals according to the dual control principle:

  • Company name
  • First and last name
  • Telephone number
  • E-Mail address

This data is used exclusively for the purpose of calculating activation codes. A combination with other data or a passing on to third parties does not take place.

The legal basis of the processing is your consent in accordance with Art. 6 (1) lit. a DSGVO.

The data will be deleted at the earliest after the destruction of all components in the activation process (terminal and card reader) or until your explicit request.

Rights of the data subject: right of access, right of rectification, right to erasure, right to restriction of processing, right to object, right to lodge a complaint with a supervisory authority

In accordance with Art. 15 DSGVO you are entitled at any time to ask CCV Deutschland GmbH for access to the personal data that are collected concerning your person.According to Art. 17 DSGVO you can request CCV Deutschland GmbH to correct, delete and block individual personal data at any time.

You can also make use of your right of objection at any time without stating reasons and change or completely revoke the declaration of consent with effect for the future. You can send the revocation either by post, e-mail or fax to CCV Deutschland GmbH. You will not incur any costs other than the postage costs or the transmission costs.

You have the right of appeal to a supervisory authority if you believe that the processing of personal data violates the DSGVO (Art. 77 DSGVO). You may claim this right before a supervisory authority in the State of residence, place of work or place of presumed infringement. In Bavaria, the responsible supervisory authority is:

Information on the procedure

All of the fields listed above must be filled in completely and truthfully in order to submit the declaration of consent.

By sending your confirmation e-mail (step 3) you agree to the data collection during the later process of calculating terminal unlock codes.

The declaration of consent must only be submitted once by you personally. We store your declaration of consent and use it automatically for every calculation of terminal unlock codes. The declaration of consent can only be given by an individual, natural person.

A collective or cross-company declaration of consent for all employees cannot be given.

The data you enter here is requested and compared by our employees every time terminal unlock codes are calculated. This is the only way to ensure that we also communicate with the person who submitted a declaration of consent. We strongly recommend that you submit your declaration of consent in good time, as we cannot calculate terminal unlock codes for you without this declaration. During the consent process, you will receive an email to the email address you provide. You must answer this e-mail for verification purposes. Only then can we store your declaration of consent as completed. It can take up to 1.5 days to process the incoming declarations of consent (sending the confirmation e-mail to you, evaluating your e-mail response and subsequent data collection).  Here is an overview of the process

    Only now we can calculate terminal unlock codes for you.

    Your declaration of consent will only be processed during our hotline hours (Mon-Fri, 8-18 h). There may therefore be longer intervals between the individual process steps.
